SASTER RELIEF FUND
In response to the harmful impact on community health in the aftermath of Hurricane Charley in 2004, the Florida Medical Association Alliance established the Disaster Relief Fund. The purpose of the Disaster Relief Fund is to provide financial assistance to individuals or community health based non-profit organizations after a natural or man-made disaster. Grants are up to $1,000 and awarded by the Florida Medical Alliance Foundation.
Criteria
Grant funds may be awarded to individuals or to community health based non-profits. Applicants may be recommended by a County Alliance or individual member. For eligibility to be considered, one must demonstrate how funding will impact the health needs of the individual or the community after a natural or man-made disaster.
Two grant requests may be submitted within a five-year period. For an additional grant award to be considered within the five-year stipulation, applicants must provide a description of the continued need and include how the additional funds will be utilized.
Applications for grant funds will be reviewed and approved by the Florida Medical Alliance Foundation Board of Directors.
A written report documenting use of the grant shall be submitted to the Florida Medical Alliance Foundation Board of Directors within four (4) weeks of receipt of funds. In the event the grant funds are not used, they shall be returned to the Florida Medical Alliance Foundation in full.
The grant award will be published on the FMA Alliance website and will be included in the FMA Alliance President’s year-end report to the membership (identity of individual recipients will be withheld for privacy).

SEXUALLY TRANSMITTED INFECTIONS (STI) AWARENESS FUND
The Florida Medical Association Alliance advocates for the health and safety of children and teens. The purpose of the Sexually Transmitted Infections (STI) Awareness Fund is to address the potential issues of sexually transmitted infections (STIs) in the targeted age groups, and to support projects that provide education on the dangers of STIs and diseases. Grants are up to $1,000 per project and awarded by the Florida Medical Alliance Foundation.
Criteria
Grant funds will be awarded to Florida projects that educate children and teens about the dangers of STIs and diseases. Applicants may be a County Alliance or individual member.
Two grant requests may be submitted for a project within a five-year period. For a project to be considered for a second grant award within the five-year stipulation, applicants must provide a description of the previous project’s success and include how the additional funds will be utilized.

ARIEL GOLDMAN MEMORIAL FUND GRANT
The Ariel Goldman Memorial Fund was established in memory of Dr. & Mrs. Stephen Goldman’s daughter, Ariel, who died suddenly in May 2001 of an arteriole-venous malformation (AVM). The purpose of the Ariel Goldman Memorial Fund is to award grants to Florida projects whose goal is to improve the health and safety of children. Grants are up to $1,200 per project and awarded by the Florida Medical Alliance Foundation.
Criteria
Grant funds are awarded to projects whose goal is to improve the health and safety of children. Applicants may be a County Alliance, individual member, or community organization.
Two grant requests may be submitted for a project within a five-year period. For a project to be considered for a second grant award within the five-year stipulation, applicants must provide a description of the previous project’s success and include how the additional funds will be utilized.

COUNTY PROJECT GRANT
To encourage County Alliances to engage in community service projects the Florida Medical Alliance Foundation offers grants up to $1,000.
Applications for grant funds will be reviewed and approved by the Florida Medical Alliance Foundation Board of Directors.
Grant awards must be utilized within the project’s timeline. A written report shall be submitted to the Florida Medical Alliance Foundation Board of Directors within three (3) months of the project’s completion. In the event that the grant funds are not used, they shall be returned to the Florida Medical Alliance Foundation in full.
The grant award will be published on the FMA Alliance website and will be included in the FMA Alliance President’s year-end report to the membership.

COMMUNITY ORGANIZATION GRANT
The Florida Medical Alliance Foundation has available grants of up to $1,000 to help community organizations that support health related initiatives.
Applications for grant funds will be reviewed and approved by the Florida Medical Alliance Foundation Board of Directors.
Grant awards must be utilized within the project’s timeline. A written report shall be submitted to the Florida Medical Alliance Foundation Board of Directors within six (6) months of the project’s completion. In the event that the grant funds are not used, they shall be returned to the Florida Medical Alliance Foundation in full.
The grant award will be published on the FMA Alliance website and will be included in the FMA Alliance President’s year-end report to the membership.
